The care plan

The care plan is an individual document written together by the doctor and the family, describing the goals of the child's diabetes care, insulin doses, devices and instructions for different situations.

  • The care plan is a legal document that obliges school and daycare to deliver care according to it
  • Ask for an up-to-date care plan from your own children's clinic
  • Update the plan at least once a year, or whenever care changes significantly
  • Hand a copy to school/daycare and keep one yourself — keep the version easily to hand
